Sustained Partial Response Of Metastatic Castration-Resistant Prostate Cancer With Bilobar Hepatic Involvement Treated With Intra-Arterial Hepatic Chemotherapy: A Case Report

Abstract:
Metastatic castration-resistant prostate cancer (mCRPC) with liver metastases often has poor outcomes with conventional therapies showing limited effectiveness. This case report describes a 59-year-old man with mCRPC and extensive liver metastases who achieved a partial response through Intra-Arterial Hepatic Chemotherapy (IAHC). After progressing on standard hormonal therapy, radiotherapy, and two lines of chemotherapy, he received 12 cycles of IAHC with Carboplatin and Epirubicin, resulting in a 10-month progression-free survival (PFS). Further hepatic progression led to subsequent trans-arterial chemoembolization. Despite initial stability, disease progression occurred, leading to palliative care. The overall survival (OS) was 26 months from the onset of liver metastases. This case suggests IAHC may be a valuable option for selected mCRPC patients with liver metastases when conventional treatments fail, warranting further research.
